Hazard, Patrick W. Age 59, December 24, 2011. Beloved husband of the late Debbie. Caring father of Jim (Maggie), Joe (Vickie) and Jenny Hazard. Proud grandpa of Kylie and Alex. Loving son of Janette Totka and the late Paul Hazard. Dearest brother of Penny, Michael, Peter, Phyllis, Dennis, Debbie and the late Pauly. Further survived by nieces, nephews, other relatives and friends. Visitation on Friday, December 30, 2011 from 11 am to 1 pm at the MAX A. SASS & SONS FUNERAL HOME, with the Funeral Service at 1 pm. Interment Good Hope Cemetery.
